Prerequisites for the Mentorship:

  1. Request for Mentorship form from the Mentorship Handbook (Appendix C)
  2. Mentorship Site Information form from the Mentorship Handbook (Appendix D)
  3. A copy of the proposed mentor's diplomas or transcript **
  4. A copy of the proposed mentor's resume or curriculum vita
  5. Mentorship Learning Objectives and Duties (which students and proposed mentors put together). Students must have a minimum of six objectives and the mentors must initial each. Students may have as many as they feel can be accomplished. These must be formatted like the example in the handbook. Learning Objectives should encompass something from each course in order to present a well-rounded mentorship experience.

** If proposed mentors do not have the required degree, but have enough experience, students can try obtaining approval by submitting a Request for Exception form and providing justification for the selection.

Guidelines for Writing Learning Objectives:

  1. Learning Objectives should be formatted as in the Mentorship Handbook, i.e. each objective should list the objective, measurement tool and expected outcome.
  2. Learning Objectives must be written in an active tense as opposed to a passive tense, i.e., performing a tangible task instead of an intangible or immeasurable task.
  3. There is a minimum requirement of six Learning Objectives.
  4. Ideally, (but this is not always possible) the Learning Objectives should encompass a task from each course in the program.

Mentorship Enrollment Process:

  1. Students submit the documents mentioned above.
  2. If mentors meet the criteria, mentorship advisors are assigned to review the Learning Objectives.
  3. Each student's file is submitted to the Associate Dean for Academic Administration for approval.
  4. Once approved, each student is e-mailed a contract via an Academy e-mail account. Students must have ussa.edu e-mail accounts and must be able to access them. All correspondence regarding the mentorship will be sent to ussa.edu e-mail addresses only.
  5. Contracts must be signed by participating students and mentors, and should be notarized before they are faxed/mailed back.
  6. Each contract must be signed by the Mentorship Advisor and the Associate Dean for Academic Administration before being notarized.
  7. Once the contract has been executed, students have 30 days to register and pay fees.
  8. Once officially registered, students start accruing hours.

The Mentorship Experience:

  1. Each student must submit a brief monthly report summarizing mentorship activity and listing the number of hours accrued.
  2. After the requisite hours are completed, each student will write a comprehensive paper on the mentorship experience, according to APA format. This paper represents 10% of the grade, and must contain three article critiques related to mentorship and include references.
  3. Each mentor completes a final comprehensive evaluation of each student's performance. This evaluation counts for 85% of each student's grade.
  4. Mentorship Advisors conduct oral interviews with students via phone or in person. This interview counts for 5% of the mentorship grade.
  5. At some point during the mentorship, each student must submit a photo of him/herself and mentor.
  6. The three grades mentioned above are added together for final grade.
  7. An end of course survey is e-mailed to each student. Failure to complete the survey results in a five point reduction in grade, and grades are held pending notification of survey completion.

Guidelines for Writing the Final Mentorship Paper:

  1. The paper should be approximately 12-20 pages in length, formatted according to APA rules.
  2. Correct spelling, formatting and grammar usage are expected.
  3. The student must detail whether each learning objective was achieved and if so, how.
  4. The student must provide a brief critique of three articles pertaining to the mentorship.
  5. The student must provide an analysis of whether the mentorship prepared him/her for future employment and if so, how.
The United States Sports Academy accepts students regardless of race, religion, gender, age, disability or national origin.
